She most likely does NOT have that. But be very very sure, day by day, that she does not have that.
Well then, I would wonder what bad thing both of you have been exposed to. Maybe some toxin, in the air or in food, for example. Or a virus or other infection. Or some TikTok challenge??
Use logic to also think over the time frame. What were you each exposed to before your symptoms started, and also do the same for her. If either of you ever goes to a doctor, mention that both have odd mystery cases. They might very well be connected.

As far as a mild-to-moderate fever: a fever is the body's natural way to help fight infection. A fever is not always some bad thing to be suppressed. But on the other hand, the immune system can produce a fever when no infection is present, when the immune system goes crazy.

Or post what her meds are, and we can check if they can sometimes directly cause a fever in some people.
Orrrrrrrrrrrrrr... if she is taking powerful immune suppression drugs because of her auto-immune, then we strongly should consider that her suppressed immunity has led to having an infection. Then we worry. That can be dangerous, especially if the fever gets higher.
In the USA, most people can order a CBC test without a doctor's order. It's only around $30. But I'd guess you do not have ability to do that in Japan. Then what about a walkin clinic to get a CBC test?

There is a big difference between syncope with chest pain, as she described -- and syncope without the chest pain, as you describe now.
The 1st is likely coronary vasospasm, the 2nd is more likely vasovagal.
If she saw a doctor today, they would order a CBC test which would likely be normal (no infection), and then the doc would probably say "it's some immune system mystery". That's because of her history.
If she was okay and now got worse again, then think about anything that might have triggered it recently. Stress?
