I actually work with the court system in my county. I guess you are talking about a jury summons from the county and not federal. As some of the others have told you, the best thing to do is to call the Clerk's Office. You want to do that early before they have already excused too many to let you off. We do not require a doctor's excuse, but some county may. We would excuse you because of your anxiety issues. You need to let them know it is severe and you are unable to deal with jury duty. Believe me, if you called our office and I talked to you, I would excuse you. I have been dealing with this anxiety and separation issues for a year and half. Good Luck.
By the way, jury duty is actually kind of fun and you get paid for it, if you are able to do it.
Yes, you can get out of jury duty, but you have to contact your local Clerk of the Court to find out the procedures in your jurisdiction. Definitely do not do what the person above mentioned, ignore the letter. Certified letter? Not gonna work long-term and then you'll have to hassle with the legal system because it's a crime to not respond to jury duty summons. That's what makes this a democratic system and we have civic duties, but there are always exceptions for disabilities.
Why don't you call the courthouse and inquire about what exactly you have to do? Who knows, it may be easy enough that they take you off the list right there on the phone.
I imagine IF indeed you will be required to submit documentation, it would probably need to come from your doctor.

Ohhhhhh noo.. i dislike jury duty. i get so nervous and anxieties. even my stomach feels so anxious.. i know that feeling. i don't know if im giving u the best advice. but ill tell u my experience. Well i had gotten two letters. and well since they are not certified letters. i just say i never received any notice letter. and bomb there u have it. lol But if u want to reply u can try to get a letter from ur therapist or doc. and write to them back and say u have GAD which means Generalized anxiety disorder .. maybe u get a free pass. good luck..
