FMLA

Do anyone have experience with filling out the Family Medical Leave paperwork?
With such varied symptoms I don't know what to say I am using this time for.
My daughter has Chiari and the
appointments, tests and driving take some of my time but her persistent vomiting
and headaches with such dizziness  don't allow me to leave her home alone!
The questions don't seem right on the form. Does anyone else know?

  Hi...ur HR rep should be able to help u and should indicate which parts of the form u r required to fill out as they have several areas one is to care for a family member....

Go to ur HR and ask they high light the info they need u to fill in....I was HR and diod that for ALL the forms I handed out...

U can take the FMLA time in hrs if u only need it for an appointment at this time....do not take full days if u don't have to so u have all the time if she should go for surgery.

Just intermittent whenever she needs me to be home with her.

Are you taking continuous time or intermittent time? I think it matters which when you fill out the forms.
