These bumps can be small cysts, hematomas, mucocele (if present on the inside of lower lip) or benign growth of the lips. Since the lumps are painless and not changing in size, all these possibilities need to be evaluated.
Pain, burning, tingling, or itching occurs at the infection site before the sores of HSV1 appear. Then clusters of blisters erupt which break down. If you are having such symptoms then you need to get some investigations like sample from the sores to identify the virus, culture analysis, Tzanck smear, antigen and antibody studies and blood sampling for antibody studies done to confirm the diagnosis.
It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. Ok now I have about twenty needle size bumps on my top lip. I am freaking out! Maybe it's some dermatology problem instead of HSV? They don't hurt and they are not even red, they are in the middle of my top lip, so it's not pimples. My lips look chapped if I don't put Chapstick on. it looks worse in the morning.
And i want to note that The cold sore on my bottom lip is now mostly gone, just looks like pink skin.
