Strange acronyms

Would like to know what some strange acronyms mean in some medical notes.  Firstly I have "DNR - getting worse".  Naturally this cannot refer to "Do Not Resuscitate" particularly since in this case no such order applies.  
Also, "SIR Dr. Phan".  
Also PN/PV.  
Also IWR (with reference to liver function).  
Also D/E or O/E with regard to affect.

Hello Capegirl,

I found a medical acronym file that has some if not all of the acronyms you inquired about. For instance, O/E means On Examination; PN means Poorly Nourished; PV means Plasma Volume or Peripheral Vascular.

These are possible and are medically acceptable abbreviations.  D/E(deferred exam), O/E (on exam), IWR (I will report), PN/PV (paranigral / plasma volume), SIR (somatostatin immunoreactivity), DNR (did not report).
