Palpitations/Echo Report

22 year old male in good health. Two years ago had serious of bad palpitations which felt like a fast fluttering in chest which last 1-2 seconds and lasted for hour or so with 10 or so episodes. Since then have  had typical PVC feelings of lump in throat, stomach, kick in chest, and everything pausing for seconds followed by rush of adrealine. Twice while awakening saw black splotches, this hasn't happened in two years.  Happened when I was on Valium.

Have had EKGs, Holters, Echo, physicals, two er visits etc. etc. All results normal except for IRBBB, LAFB, Left Axis Deviaton,Short PR interval on EKG which don't always appear. Echo report showed mild to moderate LVH with possible mild Diastolic dysfunction. Was having BAD palps during echo. Report also said mild tricupisd and mitral insuffiencies. Also have a history of ASD which resolved with age, and was on adriamycin for rhabdomyosarcoma, back in 88 for 1-2 months.
Recent cholestorl test indicated total cholestorl of 98 with HDL of 39.  

Questions are:
1. Cold the Diastolic dysfunction be from the pvcs not allowing my heart to relax between beats?
2. What to make of the black splotches? Could it be from betal blockers and valium producing low BP? I have never tollerated vallium well.
3. Does anything else sound serious?
4. Any chance of a pure EP condition, with negative holter and ekgs?
5. No family history of heart disease or syncope other than palpitations.
6. What are the odds of something serious being missed? Should I move on and deal with my anxiety and hypochondria and stop pursuing more tests.

I am 22 years old and in good health;however, sometimes when i experience gas or I burping my heart beat  becomes irregular and skips a few beats. I was just wondering what might be the underlying cause of this.

fast heart rates are not a cause of LVH.

it is more common for a computer to erroneously call LAFB than to actually have it.

Just to say I did a lot of exercising in highschool and had a very high resting heartrate of 120, with the exercise I was able to knock it down to the high 80s. Been on toprol XL and Paxil for anxiety.

If the LVH was from the high heart rate, will the beatblockers help it go down along with the diastolic dysfunction. Every doctor who has seen my records has made very little if anything out of them. Also the LAFB was from the computer and the doc crossed it out saying it was only LAD.

no, the diastolic dysfunction is from your left ventricular hypertrophy.  That is what prevents your heart from relaxing appropriately.

I am assuming that the splotches where when you stood up out of bed. If that is the case, it is from low blood pressure while acutely change posture. We are more susceptible to that in the morning.  Either beta blockers or valium could contribute to that.

3. Does anything else sound serious?

I would want to see the EKG.  LAFB is often over called on EKGs.  That by itself does not pose an increased risk though.  Sometimes there is clear explanation for LVH and sometimes there is not.  If you do not have hypertension, I am not sure why you have LVH on echo.  

There is nothing particularly high risk, but you do have a couple abnormal findings for someone your age -- LVH and LAFB.  Beta blockeer for the LVH and palpitations and treatment of hypertension if you have it is probably all that would be done for now from what you describe.

These are only brief glimpses of time.  They can miss things.  If I am suspicious that I am missing something for a patient that I see in clinic, I order a test that monitors the heart for a longer period of time.

What I usually recommend is to get two opinions without telling either doctor what the other doctor said.  If they both agree, you are pretty safe.  If they drastically disagree, it is time for third opinion.  Don't be afraid to research the doctor too.  Training can make a difference.  That being said, some of the smartest doctors I know are family practice guys that went to small programs.
