I am 30 years old. I have been inactive carier of Hepatitis B for several years with negative PCR and normal LFT and negative HbeAg until 2014. In Nov 2014, I had HBV DNA 51800 copies per ml, LFTs were normal and AFP resulted in 11 IU/ml. I started Entecavir 0.5 mg daily. At this stage, I also got HbsAg Quantitative and value was 393 IU/ml. I am continuing Entecavir since then (almost 3 years). Last year, I developed gallstone and I got laproscopic cholecystectomy done also. At that time LFTs were normal also.
Two weeks ago, I started feeling mild pain and severe discomfort in upper right abdomin leading towards back. I thought it to be something wrong with liver. I got Ultrasound and it showed normal, then I got LFTs they were normal (ALT 16, AST 15, Bilirubin 0.5 etc.). CBC showed slight increase in Lymphocytes over Neutrophils and platelets were 236,000. I went to my surgeon and he said that ultrasound seems to be normal. I went to my Hepatologist and he said that everything is ok, it may be a muscular pain. He advised PCR. I got Quantitative PCR done last week (after three years) and it showed 13 IU/ml. Then I got LFTs again and they were normal, and CBC showed 4.1 Lymphocytes (further increased) and 151,000 platlets (further decreased in 4-5 days). I was worried, then i got HbsAg quantitative and it showed 452.87 IU/ml. 4 days later I again got CBC done and it showed that lymphocytes were further increased to 4.7 while platelets improved to 300,000. I generally feel weakness in body. Now pain is little bit less but i am worried of my current condition. I am continuing the Entecavir. I am afraid of liver cancer or other such complications. I would request your review of my HBV status and give your expert opinion as to what I need to test again and any changes in treatment if required. Thanks.
