My question to my doc was regarding Sovaldi.  I did 12 weeks of Sovaldi plus peginteferon and ribavirin.  I was UND, but relapsed by week 4 post. We plan to retreat in a couple of months with Ledipasvir/Sovaldi.

Both drugs (sof and oly) probably create mutant strains that t are not very fit.

I read somewhere that it's Olysio that creates viral escape variants (mutated virus), not Sovaldi. Is that worng?

I asked my hepatologist just this very question.  The answer I was that failing treatment with Sovaldi will generate mutant viral strains.  The thing is, unlike bocepravir and telaprevir, the mutant strains are not very fit, and will die off quickly.  This, of course, means retreatment with other cocktails containing Sovaldi is possible.

Being undetectable by week 4 with this treatment has no correlation to SVR rates. Only 57% of patients are undetectable at week 4. But 93% attain SVR.

http://www.jnj.com/news/all/Final-Data-from-the-Phase-2-COSMOS-Study-of-Janssens-Once-Daily-Simeprevir-in-Combination-with-Sofosbuvir-Presented-at-The-International-Liver-Congress-2014-of-the-European-Association-for-the-Study-of-the-Liver-EASL

"Rapid virologic response (RVR, defined as undetectable HCV RNA at Week 4 of treatment) was not found to be predictive of achieving SVR. In patients receiving simeprevir and sofosbuvir alone for 12 weeks, 93 percent achieved SVR, while 57 percent achieved RVR."

Hi Fred
I failed Sovaldi Olysio I was UND during treatment but the virus back at the 12 week post the only test I did was the 12 week so I don't know how long it took to come back. I have an appointment 11/3 to see if I can get set up with Sovaldi ledipasvir as soon as my prescription coverage will allow.

I have heard Olysio and ledipasvir are different although I am not familiar with the particulars. I have also seen some of the data for prior non responders with cirrhosis genotype 1a. From what I have been able to find it looks like about 84% who's his similar to Olysio Sovaldi just hoping it works this time that ledipasvir is different enough from Olysio. But if not there are other different treatments coming later. And if need be I will be first in line.

Good luck Fred hope you are done after this treatment

When were your first labs done? I've heard some say they showed the virus still there for quite awhile and yet they went on to achieve SVR.
I've been wondering if Sovaldi and Ledispasvir are really that different than Sovaldi and Olysio, and if there's been any trials to show the first works when the latter doesn't. I doubt it. Regardless, Sovaldi/Ledipasvir should be on the insurance formularies by the end of November if it is approved by the FDA by October 10.
