Vistaril cream and Hydroxyzine tablets...at one point a medrol pak, it was all that helped.

I had itching and no rash, but it didn't move around. It was always on my lower back. I used Gold Bond Medicated cream (green bottle, red writing). It helped cool the area.

Bruising easily is not uncommon during treatment but I was wondering what stage of fibrosis you are and how your labs are looking since that didn't happen to me until I was further along during treatment. Since that is a little unclear I encourage you to contact your doctor about this.

Just because you don't have what looks like a rash right now does not mean it won't progress. I think you should call your doctor about this too. You might have to ask for a referral to a Dermatologist if your doctor does not have productive suggestions.
I did not get the Incivek rash but Ribavarin comes with its share of side effects and I ended up needing both an oral and a topical throughout treatment and after.
Have you considered taking an oral antihistamine like over-the-counter Benedryl? I did not get "the rash" during treatment but I took Hydroxizine for irritated skin and it helped a lot. It can be sedating so if that is an issue but you don't need a high dose.
I'll be honest I am not impressed with the 1% over-the-counter Hydrocortisone when it comes to things like this but you might want to give it a try - if anything to say you did if your doctor suggests it. I know you mention lotions but you might want to try Gold Bond or Eucerin if you haven't (since your doctor will very likely make suggestions like that as well).
I ended up needing Clobetasol (a very potent steroid) on my feet and knees and the relief was instant though it has taken some time to heal. My arms itched on and off throughout treatment and nothing worked for me either. I know what you mean about wanting to use an ice pick. I made the huge mistake of standing under hot water. The burning pelts against my arms felt so good. Of course it made things considerably worse. The clobetasol was a life saver.
Another steroid I see mentioned on here is called Fluocinolone. I use the solution on my scalp and it helps a lot.
When you call your doctor explain everything you did on here so they know you tried the conservative approach first and it did not help.

One thing that helped me sleep was a spray on Benedryl for kids, for sunburn. They sell it at Walmart. This got me about 3 hours of sleep per application. Also, ask your doc for a systemic antihistamine, like hydroxzine, it helped me a lot. Good Luck Mark
Also, keep showers to a minimum and don't use too much hot water. Put moisturizing cream on probably twice a day and especially after you shower.
Use moisturizing CREAM. Ask for a non greasy one. You may need a prescription cream to use at the same time for a bit. And cut your fingernails short and DON'T scratch if you can help it.
