Fertility work up done

I am 43 years old & I don't have children, but I have always want them.  At least one. Yesterday, I saw a RE and had a ultra sound of my uterus.  He advise during this procedure that he found a fibroid (mushing type) in my uterus... which would need to be removed prior to even trying to conceive. He advise my ovary were small ( which I have research can be age related).  In addition, he dictated notes to his assistant.... he said I had one follicle on my left ovary.  What does this mean?  During, the procedure I thought I understood that it was just an indication of ovulation in left ovary.  However, upon research the internet it seems there should be many more follicle on ovaries than just one.  Could this be indication of a low egg supply?  I will not get AMH & FSH and other hormonal testing results until May 2.  I have an MRI to get better indication of the fibroid location & size.

In addition, the ultra sound performed by the RE was done with an empty bladder.  I had an ultra sound of my uterus done at the end of December 2013 where the examiner required the bladder be full.... according to her that is the best way to get a clear view of uterus.  Why did the RE require an empty bladder.
