Back pain with feeling of something in throat

Hi all,

I have been having pain in my upper right back.  Between my shoulder blade and spine mostly but it has been radiating to my right side and rib cage recently.  I do not have any respiratory problems such as coughing or shortness of breath.  The pain does not get worse with activity or food nor can it be made better it is just there as a nagging sometimes sharp pain.  I have noticed it gets sharp when I burp which I have been doing a lot lately.  I have had EKG, chest x ray, base line blood work and gall bladder ultrasound which all came back normal.  I do smoke and am attempting to quit.  Have went from a pack a day down to 4 or 5 a day.  I am 29 years old, female and am basically in good health other than this.  Due to my smoking I cannot help but be afraid of lung issues. Can anyone shed some insight?
