Ok, today did me in with this office. I have been super nice and patient. Got there for my EEG and waited for an hour. Ok, no problem. It happens. Then I get to the back and the girl cannot figure out how to work the machine. I sit in the chair as she calls (on the phone) various people. She gets some other random office staff to try. After 45 minutes of sitting quietly in the chair, I say, "ok, maybe I should just reschedule." Oh, ok, she says. I reschedule and am out the door. It may be a mistake, but I called my PCP and asked for a new referral. I may have to wait, but I do not feel comfortable.

From a retired Medical Office manager, call the manager and request she/he look into it. Instead of you trying to bounce everything back and forth let them do it... They should have everything documented and should be made aware of any changes that take place in scheduling (they are usually the last to know). They should have a rap pour with the facility to straighten everything out! Good luck :)

Lucky for me, when my Dr wants any kind of test he submits a request to the lab or hospital or clinic, whatever, and they (the person who schedules whatever test is requested) contact me dirrectly to set up an appointment.
When I am contacted to set up a date I always ask for a name and contact phone number.
I believe this is standard proceedure in my province, and it works very well.
The last thing anyone needs is confusion and incompetance when dealing with health issues, and I'd be fit to be tied if I had to deal with the nonsense you're stuck dealing with.

The doctor is probably not even aware of any of this. He probably would be no help. I would ask to speak with the office manager or business manager. I had a similar problem once with my PCP. Keep your emotion out of it. Form things in a question like you are really confused. You need the staff, no point in getting anyone ticked off at you. I got labeled as a "problem patient" and the staff basically ignored my cancer for two years. Now they treat me like royalty. I was nice about it and now I get what I want when I want. They had decided I "Exaggerated" until I was finally diagnosed with cancer. Now they realize I am not a complainer but the opposite, so if I call its important. When there are multiple tests it can get confusing.
Hang in there it is irritating to say the least.

Yes I would call back, straighten it out, and request that all of these appt dates to be emailed to you.
