cervical spine mri

Can anyone tell me what means?
MRI-3T CERVICAL SPINE W/O CON
HISTORY: Left-sided numbness
TECHNIQUE: MR imaging of the cervical spine was performed without IV contrast on a 3.0 Tesla highfield
wide-bore magnet.
COMPARISON: None
FINDINGS:
BONES: Vertebral body heights are maintained. Marrow signal is within normal limits.
CORD: No lesions or signal abnormality.
DISCS: See details below.
C2-C3 :There is no disc bulge, thecal sac compression or foraminal stenosis.
C3-C4 :There is a large left paracentral disc extrusion extending inferiorly behind the C4 vertebral
body and compressing/impinging upon the left hemicord without frank cord signal abnormality. See
series 2 image 10 and series 5 image 11. The extrusion measures approximately 1 x 0.6 x 1.3 cm
(transverse by AP by CC). There is no significant foraminal stenosis.
C4-C5: There is a tiny broad-based left paracentral/foraminal disc protrusion resulting in mild left
foraminal narrowing.
C5-C6: There is a small broad-based central disc protrusion which flattens the ventral thecal sac
without contacting the cord. There is mild bilateral uncovertebral spurring resulting in mild bilateral
foraminal stenosis, slightly worse on the left.
C6-C7: There is a small broad-based central disc protrusion flattening the ventral thecal sac without
contacting the cord. There is no significant foraminal stenosis.
C7-T1: There is no disc bulge, thecal sac compression or foraminal stenosis.
The visualized posterior fossa is unremarkable. A 7 mm cyst is noted in the right lobe of the thyroid
gland.
IMPRESSION:
Large left paracentral disc extrusion at C3-C4 extending inferiorly behind the C4 vertebral body and
compressing/impinging upon the left hemicord without frank cord signal abnormality. See series 2
image 10 and series 5 image 11.
Additional mild degenerative changes as discussed above.
No evidence of demyelinating disease in the cervical cord.

I don't know what I'm reading but thinking the worst. Just started to look these things up on the internet. Scary stuff out there. Dr. Didn't mention demyelination diease.  He showed me a small spot in white matter. I had  nerve tests all good.  Go at the end of the month for evoked potentials tests ....In July going to see ms specialist.    Thank you for your input...

I was a little confused, you've not mentioned 'demyelination disease' in your posts to the MS community before........"Had brain mri done some spots but Dr. doesnt think its MS but gave me a 30% chance it could be."  so in trying to find what i'd missed i looked through your posting history and did find you've mentioned your brain MRI in your status and the neurology forum........

"I am very scared. Please help, what does this mean....IMPRESSION:
No acute infarction, intracranial hemorrhage or mass lesion
Scattered mild FLAIR hyperintensities in the periventricular and subcortical white matters with migraines, psoas ischemic change, demyelinating disease."

The noted impression is fairly standard language (missing a few words here and there?) that's used with MRI's. Usually if demyelination is evident eg highlighted with contrast, multi locations consistent with MS etc , it would not be typical to mention any other alternate conditions.

I think the listed conditions Migraine, ischemic changes (ischemic vascular disease), not sure how psoas relates to the brain?, and demyelinating disease are only the 'possible' conditions being recommended for your neurologist to consider.

Is it possible you have misunderstood and are misinterpreting the impression as meaning that you have demyelinating disease?

Cheers............JJ

Hi there,

I've looked at both your posts on your spinal MRI's and from my lay person's understanding of your MRI findings, there is no demyelination so you don't have any spinal cord lesions, which is what you'd be looking for in relation to MS........ BUT there are a few structural disc issues and cord compression noted, as well as a "A 7 mm cyst is noted in the right lobe of the thyroid
gland."

It's highly possible that these results will be your explanation, MS has many mimics and structural damage of the spine, can cause similar mimicing sx's to MS spinal cord lesions.

Cheers............JJ
