ACL shredded 2 yrs after reconstuction

I just had arthroscope surgery on my knee 3 days ago.  The dr said that my ACL was shredded and not any good. It was reconstructed with my hamstring 2 yrs ago.  What would cause this? I'm 52 yrs old.
