Extremely high TSH

I was diagnosed years ago with Hashimotos. Recently I have felt fairly well. I take an Iron Supplement. I take 150 mcg of Levothyroxine every day. My lab work is pretty consistent (give or take depending on the season). My TSH just came back 125.00000 mUI/L, everything else like the Free T4 was low normal. My RBC count was a little low at 4.06 *10 6/uL  with normal range of 4.20-5.40 *10 6/uL. My question is what in the world could cause such a high TSH number?

That's high. It may be a lab error. Are both free T4 and free T3 in range?
