Graves AND Hashimoto's!?

I have been diagnosed with both Graves' disease and hashimoto's. Can anyone explain this more to me? My endo said my option was surgery IF absolutely necessary. For now, just testing every 3 mos. I have both antibodies present. TPOab is 2000. TSI is 268. TSH went from .01 to .38 (normal range) FT4 and T3 are within range on the low side.

What are the actual results of your FT3/FT4, with reference ranges?  

What, if any, symptoms do you have?

Hashitoxicosis is a hyper phase of Hashimoto's and with FT levels low in the range, it doesn't sound like you're hyper, in spite of low (but rising) TSH.  

Sounds like Hashimoto's is dominant and I might question the diagnosis of Graves, since that's based on an index of TSI, not just a raw number.  Early stages of Hashimoto's are often characterized by periods of hyperthyroidism and it looks like you might be coming out of one.

"At least 20% of patients with autoimmune hypothyroidism also have evidence either of TSHR-blocking antibodies or, less commonly, TSI."

My T3 is 86 (range 58-159). Free T4 is 0.8 (range 0.8-1.5)

My endo told me they only test FT3 for pregnant patients.

My uptake scan also indicated graves.

REPORT TSI: 268 H %
Normal TSI(Thyroid Stimulating Immunoglobulin) <140% baseline

This is an excerpt from European Endocrinology: Hashitoxicosis – Three Cases and a Review of the Literature...

"It should be pointed out that, especially in the US literature, the term ‘hashitoxicosis’ is sometimes used to describe an autoimmune thyroid disease overlap syndrome of Graves’ and Hashimoto’s disease.2 In this article the term is strictly limited to the ‘leakage’ symptoms of active Hashimoto’s disease."

Hashitoxicosis is also the term for Graves and Hashimoto's thyroiditis combined.

Treatment for Hashitoxicosis is block and replace therapy which is taking anti-thyroid drugs along with thyroid hormone replacement.
