Ultrasound report analyses:
R lobe  volume 33 ml (per Brunn et al) this is about 10 times average (3.7ml) max 9.0 ml
L lobe  16.5 ml, average (3.7ml)
he isthmus measures 0.92 cm (average 2.3)
Total volume is 49.5 average 7.4 max 18
Heterogeneous in texture means the tissue is coarse grained
Diffusely increased vascularity of the thyroid - whole gland has high blood flow rate
Hypoechoic 1.6 cm nodule with the superior left thyroid lump or tumor with 20% chance of malignancy but can be also inflamed area
Based on the description thyroid it can be inflammation or Graves disease
Next steps: blood test for TSI antibodies; if negative technetium uptake is needed and test for TPO antibodies

Please post the reference ranges for the T3 and T4.  Ranges vary lab to lab and have to come from your own report.  Also, please tell us if those are Free T3 and Free T4 or if they are Total T3 and Total T4.  They aren't the same tests and don't give the same information.

If Free T3 and Free T4 is not specified, they are Total T3 and Total T4, by default.  Tests for TT3 and TT4 are considered obsolete and not nearly as useful as tests for FT3 and FT4.
