Can the cyst be removed if is benign? (only the cyst not the thyroid lobe). What else can be done to shrink it?

His Anti Thyroglobulin Antibody is <20.0 ref. range is (0.0-40.0) and the T4 is Free T4 test (thyroxin) but the T3 is not saying.
The last ultrasound( he had 3) result:
Mixed cystic and solid mass lesion in the inferior right thyroid lobe, with hyper vascularity surrounding and within the solid components. No evidence of lymphadenopathy. There is increased vascularity within the lesion and diffusely within the bilateral thyroid. There appears to be small amount of tissue in the area.
Yes, he has some symptoms like muscle pain, puffiness around his eyes, very red cheeks( like sunburn), constipation and he is very skinny, even he is eating pretty well.
He is not taking any medication. I asked his endocrinology doctor and she did not recommend any.
The surgery doctor recommendations:
Observe with ultrasound, do a biopsy again in April and remove the 1/2 of the affected part of the thyroid.
I am thinking to go to have a second opinion, for me is to fast what they wand to do.
Should I start giving some iron?

Eosinophils, basophils are types of white blood cells. Elevated levels would be expected with inflammation.
Elevated TPOab indicates Hashimoto's Thyroiditis. Did he also have a Thyroglobulin Antibodies (TGab) test? Both are needed; even though his TPOab isn't all that high, his TGab could be much higher, which would be solid basis to diagnose Hashimoto's.
Red blood cell count is low normal and might indicate a need for iron panel. TSH is higher than the recommended maximum of 3.0, even if the lab uses another range. We do need the ranges for the Free T4 (thyroxine) and the T3 (triiodothyronine); we also need to know if the T3 is Free or Total. If it's not specified on the report, it's total, which is considered obsolete and of little value.
Has there been a thyroid ultrasound done? If so, what were the results of that?
Does your son have any symptoms of hypothyroidism, such as sudden unexplained weight gain, constipation, dry skin, hoarseness, muscle/joint pain, facial puffiness, particularly around the eyes, etc?
Is he on any type of thyroid replacement hormones? If so, which one, what dosage and for how long has he been on it?
