High TSH , Normal T3 T4 , 3 weeks pregnant!!

Dear Doctor,

I was detected of Hypothyroidism 6 months back with TSH 26 and T4 T3 normal. My endriconologist put me on Thyronorm 50 mg and asked me to review after 3 mnths . Upon chkng my thyroid after 3 months we found that my TSH levels came down to 0.25 ..so my doctor decreased my dosage to 37mg. Then just a month after the dosage was reduced i found out that my TSH levels bounced back to 28 .So i was put back to 50 mg. 2 weeks after that i have found out that i m pregnant and in my third week of pregnancy ....my doctor has increased my dosage to 100 mg as my TSH is still around 27.  
SO now i m worried . though i have incresed the dosage to 50 mg two weeks back its still not reflecting in the test .

TSH 28 , T3 -118.9 , T4- 8.85  and 3 weeks pregnant.   dosage increased last two weeks

Q 1 -  Is it safe for my pregnancy
Q2 -   Since i was already on medication will high TSH still have effect on my baby's development.
Q -3   should i go ahead with the pregnancy or terminate it as i hv read it has sever effect of baby's development
Q4  -  what is the High level of TSH when u advice mothers to terminate pregnancy.

Please advice as i m very worried .

Hi,

you are very early in the pregnancy.  It is very normal for the demand for thyroid hormone to increase.  You were already on thyroid meds when you got pregnant.  Although your TSH is high, the FT3 and FT4 are not too low.

Q1 - It is very important to properly manage hypothyroid in preganncy.

Q2 & 3: There is no need to terminate your pregnancy.  This has been caught in good time (you were already on thyroid meds, which also has beneficial and protective effect) and it sounds like you are being appropriately managed!  You and the baby should be just fine!

The doctor should test again in 4 weeks and then readjust meds as necessary.  TSH target it 1 - 2 and FT3/FT4 should be in the upper half to top of the reference ranges.

The fact that the FT3/FT4 were in range is further reassurance that baby will be fine.  Just keep on top of this.  Keep monitoring and be anal about taking your meds properly and monitoring.

Q4 - as far as I know  there is no high level TSH at which termination would be recommended.  It would be taken on a case by case basis.  More alarming would be if the FT3/FT4 were very low.  But yours were not so low.  As you are only 3 weeks pregnant and already managing this (with the med increase) you should do fine.
