Letter to doctor

Last week I read a letter on this forum someone (sorry I can't remember who it was) had written so we could print it and take it or send it to our Dr's saying what test should be run as well as talking about treating our symptoms instead of just going by the numbers on the test. Someone please tell me where I can find it again so I can print it.

That's not the one I was looking for. It had different test that should be done like FT4, FT3, Reverse T3, B-12, ferritin and more. I'd like to show this letter to my husband and maybe his DR. He has a lot of symptoms but a couple years ago when tested for TSH and maybe FT4 the test came back normal. Hope we can find it.

One correction to note in the letter.  In the last paragraph, I left out the word not in this sentence.  

"I assure you that I have (not) selectively chosen this info, just to support my request."
