Hypo or Hyper Thyroid

My wife had been on synthroid 150mcg for many decades.  It was dropped the last year down to 112 mcg and then added Cytomel  25mcg.

Current readings:  TSH = 0.01, FT3 = 4.0, T3 = 161, T4 = 1.3   Is she in a hypo or hyper state and what should be lowered or increased?

I should also have asked if she'd taken her T3 before the blood draw.  If so, how long before?

My first thought was that was a huge increase in cytomel. Many people can't tolerate going from no T3 to 25 mcg.  They have to work their way up.

They rule of thumb is that T3 is FOUR times more potent than T4.

If that is true,  25 mcg would equate to 100 mcg of T4.

So the dosage from 150 mcg T4 to the new regime of 112 & 25 would equate to an increase if straight T4 only of 212 mcg.  Or a 75 mcg increase.  Most common increases in T4 usually is not more than 50 mcg.  And when on a fair dose already it is not uncommon to not increase T4 by more than 25 mcg and even common for 12.5 mcg increases.

Plus it is important to know if your wife is splitting the T3 dose into two doses. If taking it all at once and from nothing is creating a massive rush in the morning.  

T3 is FAST acting. Meaning that it peaks in your blood about 2-4 HOURS after taking it and then wears off and is used up completely in about 8 - 12 HOURS. While T4 takes up to 6 WEEKS to stabilize in your blood.

The bottom line is that I think your wife could have been put on a roller coaster of hell by this significant of a dosage change.  As a result it is very difficult to know what exactly is happening and how that relates to what she is feeling.

if taking all the T3 at once she probably or could likely have within hours hyper symptoms. But then as the T3 wears down through the day she crashes and has Hypo symptoms.

Just my opinion.

What are the reference ranges on her FT3 and FT4?  Ranges vary lab to lab, so you have to report them with results.
