Subclinical thyroid problem

I have been carrying thyroid function test every year in July. Since last 6 years my TSH is varying as below -starting from 2012...- 3.19,9.42,7.91,4.52,6.85 and in 2017 it is 9.36. All these years T3 and T4 is in normal range. I am not on any medicine.. Suggest whether I take 25 mg dose of medicine or just wait again...

How do you know it will cure thyroid problems?  Second you haven't confirmed a thyroid problem yet.

Ujjayi Pranayama is also known as Ocean breath or Victorious breath
is the best pranayama for thyroid problems and also to cure all throat diseases.Sit in a comfortable  pose . Take long, deep breath with both nostrils along with contraction inside the throat which makes the breath long and thin. The friction of air should be felt inside the throat when it is contracted and a hissing sound of breath is produced itself. Now exhale out very slowly. Repeat this process three times daily.

Your FT4 is at 41% of its range and your FT3 is at 48% of its range, which seem to be adequate for you since you report having no symptoms.  Your latest TSH result did not continue prior rising trend, so I would not worry about that now.  If the prior trend for TSH shows up again you could test for TPO ab and TG ab, to determine if the TSH levels are due to early stage of Hashimoto's Thyroiditis, but in the interim, there is no need to do further testing.  Just enjoy feeling well.

Thanks very much and relaxed.  However,  I must tell you that since last two months I have been regular to acupressure and pranayam related to thyroid disorder.

FT3 : normal range 1.7-4.2
Ft4 : normal range 0.7-1.8
TSH : normal range 0.3- 5.5

TSH is a useful indicator of thyroid status only at extreme values.  Your TSH has increased over the last 3 years, but at those levels it is not yet diagnostic of being hypothyroid.  Far better indicators of thyroid status are symptoms and also levels of the biologically active thyroid hormones, Free T4 and Free T3.  Your T4/T3 tests were totals, which are a measure of all your serum T4/T3.  Most of the total serum T4/T3 is bound to protein and thus rendered biologically inactive.  Dependent on the level of Thyroxine  Binding Globulin (TBG),  Total T4 and Total T3 may not accurately reflect Free T4 and Free T3 levels, so you need to know the Frees.

If you have no symptoms I see no need to start on thyroid med at this time.  Note that a small dose like 25 mcg would not add to your serum levels.  Thyroid med will lower TSH and thus reduce output of natural thyroid hormone.  Serum thyroid levels are the sum of both natural thyroid and thyroid med, so the net effect is negligible until the dose is high enough that TSH becomes suppressed enough to no longer stimulate natural thyroid output.  At that point further increases in thyroid med will start to raise FT4 and FT3 levels.  

What I would do for now is to respond to the TSH increase by testing for the antibodies of Hashimoto's Thyroiditis, to see if you are in the early stages of that.  Those tests are TPO ab and TG ab.  Also it would be a good idea to test for Free T4 and Free T3.   If you can get those tests done, please post results and reference ranges and we will be happy to help interpret and advise further.

Many thanks. To start with,  Will do free t4 and tsh test.
If you have to choose only a few tests,  I would choose Free T4 and Free T3 and skip the TSH test.   Those are the biologically active thyroid hormones.  
I have carried out test as suggested . Results -  Free T4 as 1.15 ng/dl. Free T3 as 2.9 pg/ml.TSH as 4.9. All are within limits. Thanks

What symptoms if any do you have?

Your TSH alone all those years would indicate being Hypo (low) thyroid.

Being in the "normal range" means almost absolutely NOTHING.  Especially if you are not at least 50% of the range for BOTH the Free T4 and Free T3.

Your note did not specify if the T4 and T3 were "free" or "total".

Total T4 and Total T3 are obsolete tests of very limited value.

Please indicate your test results and the reference range for the last say 2 or 3 tests.  With these results along with your symptoms would allow us a better opportunity to give you some advice.

FYI: 25 mcg of T4 medication is a standard "starter" dose.

Many thanks. My T3 and T4 is not free. The results of last three years starting from the current one is TSH 9.36, T3 117, T4 7.6.  2016 values - TSH 6.85, T3 96, T4 8. 2015 values- TSH -4.52, T3-126, T4-9.3. T3 is in ng/dl and reference range given by lab was 60 to 200. whereas T4 reference range was 4.5 to 12.


As such there is no symptoms.  I am 47 years of old.  Weight is constant around 70 kg and height is almost 180 CM.  However  since last couple of years my total cholesterol is in the range slightly  beyond  normal value of 200. Say 206,211.
