Test Result Advice

Hi,

I am a 52 year old female and I had been on 15MG Methimazole for the over 2 months and Endo lower the dosage to 10MG when my Triiodothyronine (T3) was within range.  I am currently taking 10MG of Methimazole and had been on this dosage for 12 weeks.

TSH                             4.02              Range  0.45 - 4.5
T4, Free                       1.07           Range 0.82 - 1.77
Bilirubin, Total               1.3   High     Range 0.0 - 1.2
A/G Ratio                      2.3   High     Range 1.2 - 2.2
Triiodothyronine (T3)    85                 Range 71 - 180
T3, Free                        2.9               Range 2.0 - 4.4

I am sleeping a little better (4-5 hours intermittently), but still wake up at 2-3 am with difficulty falling back to sleep again.  I will then take10MG of doxepin to fall back asleep. I am wondering when my body will resume back to normal sleeping?  I still battling with anxiety and started prozac 2 weeks ago.

I will be seeing my endocrinologist next week, and would like to know if there is anything I should be concern with and if I need to ask any questions.

Thanks!

I think that you are grasping at straws thinking it might be white blood cell related.  Hypothyroidism can cause  huge number of different symptoms.  Note that hypothyroidism has been reported to cause any of the following symptoms.

Emotional Related:

Tension
Irritability
Wanting to be solitary
Mood swings
Anxiety
Personality changes
Feelings of resentment
Jumpy
Easily startled
Lack of confidence
Nervousness

So the first thing I suggest  is to get your doctor to adjust med as needed to get your FT4 and FT3 levels up to around the middle of their ranges, and note the effect on how you feel.  Also, I suggest testing for Vitamin D B12 and ferritin.  They are important for a thyroid patient.  D should be at least 50 ng/ml. B12 in the upper part of its range and ferritin needs to be at least 100.  If you want to read about this, following is a link to a paper I co-authored.

The only thing I can suggest is that your thyroid levels are a bit lower than needed by many people.  Your fT4 is only at 26% of its range, and your FT3 is only at 37% of its range.  Being a bit hypothyroid can affect sleep.  So, you might ask the Endo about lowering your dose of  Methimazole.

I still don’t understand why I am still having anxiety.  Do you think I should ask my Endo to check my white blood cell count?  I heard in rare condition that methimazole can decrease whistle blood cell and lower white blood cell can effect moods.

Thanks!
