They are going to biopsy both nodules because they are over 1cm and I have family history. What does the isthmus being too thick mean? I am female do you know what the typical volume is for female? There was some lymph nodes in my neck that were enlarged but they did not check them during ultrasound, my adenoids are also enlarged which I understand is lymphatic tissue, does that increase malignancy chances? Typically the ISOechoic nodules have low risk for malignancy, so at the worse case only the 1.74 nodule should undergo biopsy. If no abnormality is found in lymph nodes the risk of malignancy is very low.
Both lobes demonstrate uneven texture due to the past or present inflammation.

the size can decrease after acute phase or after excluding inflammatory triggers form the diet
