Thyroid Results Normal but....

Have symptoms of both hypo and hyper.... my mom was diagnosed with hyperthyroid.... d. said everything is normal....just don't feel right...

test results
esr is 11 range .20 mm/hr
tsh 2.858 range .350-5.500 ulu/ml
free t4 1.11 range .89-1.76 ng/dl
free t3 3.4 range 23-4.2 pg/ml
anti tg  41
anti tpo 44 range <61
tg 12.3 range <55

I'm happy that your Subacute Thyroiditis was discovered and you were able to get relief for you symptoms.  It sounds like you have hyperthyroidism.  Have you been tested for Graves Disease?  That's what all of your symptoms and thyroid test results indicate.

My TSH was <0..005, Free T3  8.7, FreeT4  5.3  and ESR 100, CRP 64, WBC 19000 and Platelets 490000 all very high. Fever. weight loss, tachycardia, extreme weakness. Tested Negative for all infectious diseases. Thyroid scan (Radioactive Iodine uptake) confirmed Subacute Thyroditis. I was told to avoid Iodised Salt and just one medication i.e. Propranolol 40 (Beta blocker) which cured me and I got tested my Thyroid Profile last week and it was normal. Fever gone, started gaining weight, tachycardia gone.

I just got almost cured for Subacute Thryditis but it played havoc with me. I landed twice at hospitals for extreme sweating and tachycardia as if it were heart attack or stroke. I suffered from almost all worst symptoms of S/A Thyroditis. Fever for almost 2 months, weight loss (10 lbs), tachycardia, extreme weakness, neck pain, pain behind ears but most of the doctors mistook it for some infection and kept on prescribing me one antibiotic or the other for over a month. In between they advised me to go for a surgery for a perianal infection with pus pocket which they thought was the cause of fever but even after surgery fever still persisting. It used to be between 99-100.2. Finally I landed at Apollo Hospitals Delhi and they diagnosed it Subacute Thyroditis. They prescribed me just one medicine Propranolol 40 (long acting) and told me not to use Iodised salt. I started getting symptomatic relief within 4 days of starting this medication. Fever gone away and I started feeling better and better as the days passed by. I have also started gaining weight and I got my TSH, T3 and T4 tested last week which were normal now but doctor has advised me to get them tested agagin after a month. This disease is very rare. and confusing because of its vague symptoms.

Looks like I got in a bit of hurry, doesn't it?  My last paragraph should have read:
"It's always best to "pre-interview" doctors to make sure the next one you get won't be just as bad or worse than the one you have."

I didn't go far enough with that explanation.  You can often pre-interview doctors through their nurses or an office manager.  You want to make sure that the doctor is willing to test the biologically active thyroid hormones, Free T3 and Free T4; you want to make sure the doctor is open to prescribing all types of thyroid hormone replacement medications, including those containing T3, such as cytomel and/or desiccated; you want to make sure the doctor is open to treating by symptoms not just the lab numbers and especially not just by TSH.  If he answers to any of these questions are not satisfactory, don't made an appointment with that doctor.

Your results are in the normal, but that doesn't mean you aren't hypo.  As I noted above, it's very common for symptoms to appear before labs indicate an issue.  Many doctors are unwilling to treat symptoms.  

IMO, unless you're willing to put up with the symptoms until your labs go completely out of range, so you doctor is willing to treat you, it's time to find a different doctor, but you're the only one who can  make that decision.

It's always best to "pre-interview" doctors to find make sure the next one you get won't be just as bad or worse than the one you have.  Make sure they are willing to treat symptoms, not just lab numbers.

Sorry, I thought those were your mother's labs.  

Your antibody tests don't indicate Hashimoto's at this point, but since antibodies are present (just not over the reference range), it's very possible you do have it and it's just not ramped up enough to show on blood tests yet.  Hashimoto's is the number 1 cause of hypothyroidism in the developed world.  Ask for a thyroid ultra sound, which can also confirm Hashimoto's.

It's very common for symptoms of hypothyroidism to appear, long before blood tests will confirm.  You'll have to keep after your doctor it.  

High cholesterol is another symptom of hypothyroidism.

Some other symptoms can apply to both/either hyper and hypo.  Depression and anxiety are some that do.  Fatigue is another, as are heart palps, shortness of breath, etc. I had them all when I was severely hypo.

If your doctor won't listen, it may be time for another.

thanks for your quick response... those were actually my lab results..I have felt UN-normal for a while and belied i have thyroid issues.... been on meds for anxiety and ocd for a while....  I have both symptoms of hyper and hypo....just used my mom as an example that it runs in the family.... my dr insists there is nothing wrong with me! I recently broke out in a crazy rash....no one could figure out what it was or what caused it. I was not exposed to anything new, ....have terrible mood swings, always cold, at times no energy and the list goes on. my dr says its part of getting older....I just know its something more.....

When was your mother diagnosed with hyperthyroidism and on basis?  Is she on anti thyroid medication of some kind?  Her results actually indicate that she's hypo.

Her lab is using an outdated range for TSH - the "new" range determined almost 12 yrs ago is 0.3-3.0, but labs and doctors have been very slow to embrace it. Your mother's TSH is very near the top of that range.

Her FT4 is only at 25% of its range; rule of thumb is for FT4 to be about mid range.  Her FT3 is at 57%; rule of thumb is upper half to upper third so she's in the ballpark there, but results are far from hyper.  FT3 is the hormone that correlates best with symptoms.  High FT3 would indicate hyper and low FT3 would indicate hypo.

Exactly what symptoms does your mother have?  Some symptoms can cross over i.e. apply to either/both hyper and hypo.
