You need to be tested for thyroid antibodies to determine if your hypothyroidism is caused by Hashimoto's. That's an autoimmune disease, in which your body sees your thyroid as foreign and produces antbodies that destroy healthy thyroid tissue, so it can no longer produce hormones. As the antibodies destroy the thyroid, hormone production is less and less, so medication dosage has to be increased periodically, to take up the shortfall. The antibody tests you need are Thyroid Peroxidase Antibodies (TPOab) and Thyroglobulin Antibodies (TGab)
Just having hormone levels "in range" isn't good enough. Your Free T4 is too low in the range. Additionally, Total T3 is obsolete and of little value. You should ask to be tested for Free T3 instead, as that's the hormone that's actually used by individual cells.
AM cortisol, by itself isn't very helpful; you should see about getting a 24 hour test that shows cortisol levels over course of the whole 24 hrs.
Does your lab report differentiate between age groups for the androgen index and DHEA? People of different age groups would have different levels; it's normal for DHEA, in particular to decrease as we age.

Looking at your labs, I'd say you need an increase in Synthroid.
Do you have Hashimoto's?
Type II diabetic on metformin. I don't understand the calculation for the blood sugar (?) i.e "5.5 means X 18 =95". Please explain that. I'm in the U.S and we might have different ways to calculate blood sugar levels.
