If you had a proper abdominal surgery then the cause could be adhesions of the surgical site with other internal organsOrgan-1 nr of the abdomen.
The pain due to adhesions can be a dull ache, a shooting pain, a pain as though your intestines are being pulled out or a pain that appears to rip the abdomen apart. The type of pain that develops depends on the degree of adhesions and the internal organsOrgan-1 nr that are feeling the pull of these adhesions. The only way to diagnose them is through laparotomy.
The other possibility is infection just below the incision site. It is difficult to comment beyond this without examining.
