Dear Doctor,

I do not ejaculate or have great difficulty ejaculating during intercourse.  This is not a problem during masturbation.  I am 62 years of age and am not on any medications.  

The makers of Viagra, Levitra etc claim that as a side effect their product counteracts premature ejaculation.  Is there a medicine, cream etc. that, taken orally or topically applied, has the opposite effect, that is to precipitate or hasten ejaculation?  Or a cream etc. that, topically applied, increases sensitivity of the glans (glands).

I am interested to have your advice.

I would suggest further testing to ensure there are no anatomic abnormalities.  A urology visit can ensure that the pulses and the vasculature of the penis is intact.  A penile ultrasound can be considered to evaluate the vasculature.  I would also test the total testosterone level to ensure proper levels - which can affect ejaculation.  

Without knowing the cause of your symptoms, it is diffcult to recommend treatment.

I would suggest a urology referral to discuss the aforementioned tests.
