pain before bowel movement

i get this terrible pain at the under side of the penis when the bowel mussles contract just before bowel movement. i don;t dare to relax mussles afraid of an accident. but pain goes away after movement and does not happen all the time. but when it does happen ik can barely move. any help would be appreciated
