Go to the drug store and buy a drug test kit. You can probably get your answer just by waving the cup in his face. It's hard to say which is which because the signs are almost true for both. Water retention from Methadone will cause some swelling in the hands and feet, and if the dose is too high, he will nod off quite a bit when he is on it. I never took heroin, but I volunteer at a detox facility 3 nights a week and have been in as a patient numerous times till finally becoming sober. I have seen the damage and spoken with countless heroin users and they all have the same stories.
Some of us talk and talk like we want to quit but need that extra push from someone. I would start with the drug test. Watch the reaction when you confront him. His guilt or innocence could be determined without even making him take it. If he volunteers then maybe he is just taking too high a dose of Methadone. Another thing I experienced while taking it along with swelling was occasional numbness on those same parts.
It's not me. We found out several months ago that my 30 year old son was addicted. He has been fighting it with methadone. I noticed his fingers swelling up and am worried he is using herion again. He was injecting it into his neck and now he is keeping his neck covered up so we can't see it. Before I ask him to take a test I want to know if the swelling is a sign of use.

You didn't quite provide enough information so I'm going to guess once again. You shooting heroin? Specifically the hand area? Doesn't really matter but excessive needle use damages the veins and causes poor circulation. Don't want to state the obvious, but if you desire any kind of recovery to your veins, you should stop. Continued use can lead to further complications and even damage your veins beyond repair. Exercise will help pump the blood some but is futile if you continue to use. I'm not a medical professional so it is very important to see a doctor if this continues.
Just for clarity, I don't know any elderly IV users, and no one is bullet-proof, so quit before you catch the serious stuff ( i.e. Hepatitis and others).
