I have alopecia areata, and the feeling of bruising seems to pre-empt hair loss in that area.
I have been experiencing tender spot on top right size of my head for the last 3 or 4 years. The tender spot felt pain to touch and I also noticed thinning hair on this sore spot. Initially I just ignored it and also found that massaging the spot would reduce the pain. I am 48 years old female, health wise I am good, no medical problem, except for I experience anxiety on an off for which I have to take anxiety medication regularly.
I then got curious about finding the reason forr this tender spot and thinning hair at it. Did a bit of exploring the net and came to this page which I really found very useful. I hope it would help all those who have similar problem.

Anxiety can and does cause hair loss, but not the sore spots. There is more going on and it may be as simple as the new shampoo. If it continues then I would see a Dermatologist, this is their specialty and your best resource. If it's the shampoo, you should start to feel a difference with the sore spots in a week or two, but it may take some time for the hair to grow back. Wash your hair gently so you don't irritate the sore spots.
Below is a reputable website that explains how stress and anxiety can cause our hair to fall out along with other causes. Good luck, I hope it's the shampoo and you start to feel better quickly.

That makes two of us that aren't impressed by her answer!
I suppose it's possible for somebody with constant, acute anxiety to suffer some hair loss. I won't give her the point, but I'll give the "possibility" some leeway.
A sore spot on the scalp from anxiety? Sorry, but that's simply too large of a leap for me to buy into.
I've been on this forum for a number of years and I honestly don't think I've ever seen anyone discuss hair loss or sore scalps from anxiety. The information you found on your own is far more logical.
If you go back to see this doctor, would you kindly ask her to site her resource that she based her diagnosis on. If this was based in science, all of us on this forum would look like coneheads....................
Let us know how the organic shampoo is working.

I saw my doctor yesterday. She said that these could be related to anxiety, even the tender spot; however, I wasn't impressed with her answer or her lack of time spent with me (she was in the room with me for about 5 minutes and the majority of time just typing on her computer), so I did a little research and found that it could be an allergy to a new shampoo. I did start using a new shampoo recently, about the time I noticed the hair loss, so I've decided to switch that out with organic shampoo (no sulfates) and see what happens.
I'll update in a few weeks if it works.
You don't say what your other health concerns are or if you're on any medications which could possibly be causing these issues. But since you are seeing doctors for tests, please let them know about the hair loss and the sore spot on your head.
Garden variety anxiety does not cause hair loss nor a tender spot on your scalp.
If you aren't scheduled to see a doctor soon, I'd make an appointment to have this checked out. It sounds like you have enough to worry about without this as well.
Please let us know what they find out, OK?
I wish you the very best
Peace
