Could have to do with Vagus nerve dysfunction, excessive stress-including hidden stress- or a combination of these and other factors.
Yes, it could be very well in your head, as the vagus nerve is the largest cranial nerve and it goes all the way down to the stomach!

There is no way your mind could give you those symptoms in your guts. You have to stop coming up with random theories like "adrenalin" affecting your guts and discuss it with a doctor who will listen to your symptoms. If your current Doc. Thinks it's psychological, consult other doctors.
You describe classic signs of anxiety, while possibly "in your head" is a very real physical imbalance of brain chemistry. I have dealt with anxiety and panic for 50 years and have successfully treated the "gut wrenching" symptoms associated with anxiety. My anxiety is hereditary and can come on for no reason whatsoever, and it definitely comes on with common stressors like fear of public speaking etc., often associate with "worry", at a more extreme end it is panic, with a "fight or flight" response in the brain. It is easily treated with medication as needed. Anxiety can't be ignored, only held in, so others don't see it. Forget that, it is a physical problem, whether anxiety or something else. Find a doctor that cares. No doctor today would tell you it is in your head without understanding the physical relation to brain chemistry, and should help you or refer you to a specialist, if it is a significant problem to you which it is since it has continued for 13 years. Untreated chronic anxiety will usually worsen with age, don't wait, fix it now.
Hello~Your symptoms sound very much like they are stress/anxiety related. True, they may very well be in your head, but, they are real to you just the same. You need to find a good, sympathetic doctor who will listen to you and prescribe accordingly.
Anxiety can cause all kinds of symptoms, I have a blog here that you are welcome to read, the symptoms are never-ending.
