Hi,

I just wanted to follow up since we haven't heard from you in a while and see if you've had any changes in your condition in the intervening weeks, or if you've been able to get an appointment with a doctor or possibly even get in to see one to address your symptoms.

I hope you are making progress and welcome you to follow up with us regarding questions or concerns you have or just to update us on your situation.
