Mirena issues

Hi.
This is my second mirena in just over a year, the first on tried to expel and got stuck in my cervix and after that I waited 2 months to have this one put in.
The issue I am having is I have had pain in my lower back/pelvic area for 48hrs now and I every time I pass urine it hurts if I push down (like giving birth pushing) and the strings are still there although I believe I can feel more of them then I could to start with.
I'm abroad at the moment and I am concerned because I don't want to be turned away from the er, but I don't want to waste anyone's time. I have just moved here and I have got a gp yet and I don't know what the wait times are for them.
I have also had uncontrollable mood swings(just generally upset, moody and anxious) and thats not like me either.
Any advice would be appreciated.
  I do realise moving will cause emotional outbursts, but these have been going on for about 7 months now. And I get pain on intercourse also now, which I never did before.
