Do I need chemo/radio/hormonal therapy?

FNAC of my breast lump showed "Apocrine metaplasia with atypia". Lumpectomy biopsy showed "Infiltrative ductal carcinoma" and MRM biopsy showed
"Remaining breast shows features of fibrocystic disease, nipple and areola free of tumour invasion, both skin margins are free of tumour ,base consist of fibro fatty tissue and unremarkable,negative for malignency.,Lymphnodes 0/17 -Negative for malignency and all nodes show reactive changes with sinus histocytosis, Lymphnode 0/1 show reactive changes. Negative for malignency,remaining fibrofatty tissue is unremarkable.
IMPRESSION:
NO RESIDUAL TUMOUR FOUND, FOCAL INTRADUCTAL HYPERPLASIA,LYMPHNODES (0/18)-NEGATIVE FOR MALIGNENCY
My bone and liver scan are normal,ERPR not detected.
In view of the above, please advise me DO I NEED CHEMO/RADIO/HORMONAL OR ANY OTHER THERAPY AND WHAT IS THE EVIDENCE BASED TREATMENT FOR MY SITUATION. i'M A MEDICAL DOCTOR RETIRED AND MY AGE IS 63 YEARS.

In any case, your oncologist would be in the best position to make treatment recommendations related to factors in your specific case,  based on his/her knowledge of all imaging, tests, the full pathology report from your lumpectomy (including size of the tumor and grade of the cancer cells, etc.),  the TNM Staging of of your breast cancer, and your medical history and current health status.

If you are in doubt about your oncologist's recommendations (or lack thereof), I would suggest a 2nd opinion consult with another oncologist.

This is a question for your Oncologist ... we here cannot advise any type of treatment. I can tell you that there would need to be as assessment of the ER/PR response before any Hormonal therapy could be considered. If positive it is definitely advised for 5 years (possibly 10) to reduce the risk of recurrence. We have an "Expert" Breast Cancer Forum where even though the number of posts per day is limited, the answers come from Physicians. Best wishes for a speedy and uneventful recovery as well as a successful treatment plan if one is recommended.
