Malignant Phylloides tumor

My sister is 31 years old. First time, it was last year in the month of November she was feel one tumor in her right breast then meet with local surgery doctor for treatment. When the doctor was suggesting the tumor must be surgery for fully removed and doctor surgery accordingly as on 06 Dec, 07. After surgery biopsy report is show, it is Phylloides Tumor. She was felling some pain before and after surgery both times. She was pregnant on the month of April 08. Unfortunately, second time same place the tumor was started to grow rapidly in the month of Aug & sep 08. Further meet with another surgery doctor for treatment and he was suggesting very recently surgery it because of already it is very big within short time. When her Plasma Glucose Random 4.2 mmol/L. By doctor, surgery on 12 Oct, 08 in second times with her pregnant situation. After surgery biopsy report as follows:

Macroscopic Description: Specimen received in formalin with proper lab no consists of a globular partly skin covered piece of breast tissue. It measures about 14x11x6 cm. The cut surface shows a grey brown nodule measuring about 8.5x6.0 cm. It is very close to the deep resection margin and overlying skin. Embedded nine blocks as follows: A1-A3 = three blocks from the nodule, A4 = one block from overlying skin, A5 = one block from deep resection margin and A6-A9 = four blocks from the lateral margins.

Microscopic description: Sections of breast show a malignant phylloides tumor. The stroma is highly cellular. The stromal cells are pleomorphic. Mitotic figures are seen frequently. Tumor necrosis: Present, Mitotic figure: Frequent, Vascular invasion: Not identified, Overlying skin: Free of tumor, Deep resection margin: Free of tumor, Lateral margins: Free of tumor.

Diagnosis: Breast lump right (lumpectomy): Phylloides tumor, Malignant.

Now, surgery doctor is suggesting after 8 months over of the pregnant then delivery complete by surgery. After complete delivery, she will take chemotherapy then will be prevent from Malignant Phylloides tumor.

As a result, as per above statement I have needed few answers of the below Ques.:

1. Have spread to the other part of body Malignant Phylloides tumor? Somewhere I read it can be distant to the lung.
2. Have any effected to the pregnant baby?
3. Has she breast-feeding for her child during the taking of chemotherapy?
4. Has she right breast-feeding for her child after delivery?
5. Are the malignant phylloides tumor an increased risk of “breast cancer"?

Please ask as many questions as you need to .. that's what we are here for. You are correct that the Malignant Phyllodes Tumors do not respond well to either Chemotherapy or Radiation but they are often used in treatment. I believe if your sister can, it would be to her advantage to see the Oncologist now and he could give her a better idea of what treatment will be best for her. Just because she can't start treatment now the Oncologist could certainly answer a lot of her concerns about what treatment would be best for her particular case.I recall you ask before if this cancer could spread to the lungs ... it also can spread to bone, brain, liver and has been known to return to the breast even two years after treatment. when other parts of the body are effected by the cancer the first indication is usually pain or with the lungs it could be some difficulty breathing or possibly frequent coughing.   Kindest regards ....

We are always meet/check with surgery doctor. Surgery doctor says that after delivery she should begin Chemotherapy. Have we need meet with Oncologist doctor? When after delivery or right now? In this web site I read that not benifit take Chemotherapy for Malignant Phylloides tumor. pls describe about this. Have any simtom, if spread to the lung and effected the baby also?

Until the baby is born there isn't really much in the way of treatment but as soon as the baby comes your sister should begin Chemotherapy as soon as the Dr.s will permit it. Is your sister in the United States or another country? Her Oncologist will be the Dr.to decide what treatment will be right for her and also what tests can be done to see if the baby has been effected.  Best Wishes ......

Now what should do for my sister and her pregnant baby? and what's treatment take for baby after delivery?

1. If this tumor does spread, it's usually to the lung.
2. This type of tumor spreads thru the blood vessels .... could possibly have effected the baby.
3. NO breast feeding while taking Chemotherapy.
4. I would not advise breast feeding at all .... even before Chemotherapy.

Malignant Phyllodes tumor is a rare and aggressive form of breast cancer ... usually a mastectomy is recommended. This type of breast cancer is known to spread more often than some of the other types. Your sister would not be able to nurse if she is receiving Chemotherapy as the drugs go thru the entire system by way of the blood stream. Being an aggressive type of cancer it can recur and often does; as it has in your sister's case. with the appropriate treatment we can hope that no cancer cells will remain and she will go on to live a long life and enjoy her new baby.  Kindest regards ....
