p.s. To answer another part of your question, there are many medications available to treat osteoporosis, and if your doctor's intervention does not lead to approval for Prolia, s/he would be the best person to recommend the alternative most appropriate for your specific case.

My suggestion would be to have the doctor prescribing, or recommending, this medication, to contact your insurance with a medical justification for an exception in your case.
Most insurances have a "special circumstances" dept. which considers such requests.
