I don't know what i'm going through! Please help!

Hello, I'm Rayan, 20 y.o,173cm, male.

I have been experiencing a lot of frustrating moments of fatigue, feeling like not getting enough air, strong heart beats, feeling unwell all day long, i sleep like I'm dying, i even wake up with weird fatigue with weird upper back and shoulders blades pain that never left me a moment in every wake-up , my chief complaint is FEELING SHORT OF BREATH! Like i cant get comfortable with my breath and it's with me all day long!

It was the day of 2014/7/23, when i was laying on my bed watching some movies, suddenly i felt like i'm not breathing well and there's a problem with my breath! Like i'm fainting and suffocating!

After that day all my health problems came together ( Constant fatigue, constant shortness of breath, stomach pain and lateral chest pains, upper back pain). And i did a tough diet dropped my weight from 82 to 58.
The symptoms came after my weight  dropping maybe 3 months after, i went to 2 cardiologists and did echo and ecg, all said nothing's wrong, but i have a mild pulmonic regurgitation that i was diagnosed when i was 14 and the regurgitation precentage is still the same. Went to a pulmonogist with physical examination and lung function device, he said that i have a mild asthma that i had since i was a child and didn't affect me. I did have a constant digestion problems and also constant acid reflux, my question since all doctors say i'm fine and I don't know what i'm going through since july 2014, can the acid reflux be the chief problem? Please help I couldn't even arrange my words i'm totally broke, i miss my old days, what is that weird fatigue im facing whether when exerting a small work or not doing anything? I forgot to say that i went to many psychiatrists they gave me some medications for panic attacks and depression, they did benefited me but not that much, maybe just 9%! Please help i am really going crazy:(

If you are taking Vitamin D on your own, make sure to get a blood test every six months.  To much Vitamin D, you can become toxic.  

For other issues try alternative medicine NAET and/or BRT,  body restoration technique with a chiropractor that does applied kenisology. AK .

Another technique is BAX 3000 and BAX AURA,  also with a chiropractor. These may be harder to find.  

All these methods are NON invasive and a higher level of accupunture without NEEDLES.  

These methods will help the body build tolerance and improved immune system.  

Quality of life will improve.

It sounds like you suffer with apnea you should seek help because of what you're saying sounds very serious if you leave this untreated for too long. I might be wrong but really tell the doctor everything that you're experiencing, even show the doctor what you are concerned with on the note you wrote here. God Speed!

I know this post is a little old, but did anyone ever recommend a sleep study? Acid reflux is a common symptom of sleep apnea as well as some of the other issues you describe. Might be worth talking with a sleep physician....

OK, Nobody here can rule out anything based on the details you posted,
plus it would not be ethical to do so.
Therefore you can rely on your doctor's opinion, or you can go and get a second opinion on the  pulmonic regurgitation.

The one thing I can tell you with certainty is that after doing a drug interaction check between Citalopram and Remeron the following came up:

Using mirtazapine together with citalopram can increase the risk of a rare but serious condition called the serotonin syndrome, which may include symptoms such as confusion, hallucination, seizure, extreme changes in blood pressure, increased heart rate, fever, excessive sweating, shivering or shaking, blurred vision, muscle spasm or stiffness, tremor, incoordination, stomach cramp, nausea, vomiting, and diarrhea. Severe cases may result in coma and even death. You should seek immediate medical attention if you experience these symptoms while taking the medications. Talk to your doctor if you have any questions or concerns. Your doctor may already be aware of the risks, but has determined that this is the best course of treatment for you and has taken appropriate precautions and is monitoring you closely for any potential complications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Serotonin syndrome could  potentially be a very dangerous condition
AND it could be responsible for your breathing difficulties,
However because of your medical history, previous diagnosis and the likelihood that your breathing difficulties might be a symptom of Serotonin syndrome which developed later, may not draw the required medical attention.

And the biggest issue you might have is that there's no lab test to help diagnose Serotonin syndrome.
Is your prescribing physician going to go out of his/her way to diagnose this????? Another doctor perhaps??? Hmm!
You see in an ideal world , your doctor after a very careful risk-benefit
assessment, would have advised you of all this, before prescribing these meds.
But we don't live in an ideal world and it may become an issue, which you may have to pursue on your own-you really need to get to the bottom of this right?
Can you perhaps offer any details on the prescription history?

IDK Rayan , but what I would do right away is to limit foods rich in tryptophan  (which is converted to serotonin in the body) and take gelatin
which also reduces serotonin, until you figure all this out.

There are tests you can do for serotonin levels but not to diagnose serotonin syndrome as I mentioned earlier.

I will have vitamin K2 and Methylocobalamin B12 sub-lingual drops
As soon as possible!

The thyriod test as i recall was normal, only the vitamin D deficiency.

I don't know what I'm facing i really don't:(
I have it everyday everytime every seconds.
I really want to reclaim my lost health and happiness.
So what is the whole problem do you think that made me like this? Fatigued with a little exertion and feeling like I don't feel good when breathing like i'm not fulfilling my need of oxygen, it's so weird I can't explain:(

I'm taking REMERON MIRTAZAPINE 30 mg tablets
And CITALOPRAM HEXAL 20 mg
I used to take xanax but my doc suggested me to decrease the intake till i stopped it.
Please tell me that pulmonic regurgitation has nothing to do with my case, i mean the constant feel of breathlessness and the immediate fatigue with pain of ribs and upper back:(

Can this be caused by digestion problem? Like colon problems and acid reflux( both i have)

That changes a few things.
You Vitamin D levels are too low. let's see...
Thyroid function depends on adequate vit. D levels (Thyroid controls metabolism and energy).and so do  B12 methyocobalamin and ferittin (low vit. D impacts the G/I system- impairs absorption of vital nutrients and vitamins, as it has most of its receptors in the G/I system).
So it becomes a vicious cycle.
In addition to this vitamin D is more of a hormone with a plethora of functions including the signaling of about 300 (not a typo) different genes.
You can see how vital it is to bring your stores up.
I would also recommend  to supplement with some vitamin K2 in order to ensure the  calcium which is metabolized by the vitamin D3, goes to bones, joints and teeth ,instead of soft tissues!
Sub-lingual drops would work better as they would get absorbed faster
by bypassing the digestive tract.
It will take a few months to see a notable improvement.

I would also consider Methylocobalamin B12 sub-lingual drops, in light of your low  D3 , since low D3 affects B12 absorption (methylocobalamin B12
is the preferred one).

My opinion is for now to try to correct imbalances and deficiencies, since your doctor said the problem isn't from the regurgitation and there's not much you can do about it at present.

What anxiety meds are you on now?

Sorry forgot to say that unfortunately after this problem that made me let sports and with the anxiety meds . I am now 83 kg

And for the name, It's okay mate I'm good with both of them!

I'm sorry i forgot to mention that i had a blood and urine test 1 year ago, yes within a few weeks after the problem. And I'm sorry I don't have a picture of it but i do recall that the lab specialist said that my RBC picture is normal and well and all the other blood substances were normal except the Vitamin D was 15 only( I'm sorry I don't know the actual scale of it) but i will do the baking soda purb and the betaine HCL challenge when i get back to home!

But my whole concern is on two things ( the heart valve regurgitation and the acid reflux)

For the heart valve, i was afraid that this might be pulmonary hypertension, but the cardiologist said I don't have it, i said that this maybe came from the regurgitation itself but i was relieved after the doctor said that the regurgitation precentage is the same as the first time i was diagnosed( mild) i was relieved because i put in mind that this precentage didn't affect me in the past (before this problem)
So am i thinkng right? And the problem isn't from the regurgitation?

For the acid reflux, could it with the a lot abdominal gases  cause the constant feel of breathlessness and the weird fatigue with the weird upper back pain? (Forgive me I'm using too many Weird words, because it's really weird feeling:( )
One last question why do i feel fatigued and breathless while i do a less work, and the fatigue stays for 30 mins after the exertion, also why when i wake up i be fatigues with upper back pain like it's inside not outside, also with chest ribs pain and of course with the chief problem (shortness of breath) :(

Know please that i'm really thankful and I'm so sorry for saying a lot!
But no one gave a real diagnosis for what i'm going through!
All say anxiety, asthma,..etc. that I don't think are the cause!

Hi Ryan.

I'm going to assume you are about 58 Kg, which BTW could be so low,
that it puts you at an increased health risk.
There are simply not enough nutrients in  a low calorie diet, to sustain you
and remain healthy.
The rule of thumb is that your weight in Kg should be the amount of centimeters from your height over 100. So that is 73 Kg.
That's just an average approx weight and  you could adjust it up or down
a bit to allow for other factors like bone structure, muscle mass etc.

--Having said this, you really need to know that you have enough cholesterol and fats in your system to allow for metabolizing  fat soluble nutrients and vitamins like vitamin A,D,E,K , the synthesis of hormones and  healthy neurological function.
--You also need to ensure your Nitric Oxide levels are normal.
Nitric oxide is lethal to bacteria and viruses and is also known to increase oxygen absorption in your lungs from 10-25 percent among many other things.
--Other deficiencies you should look into is Vitamin D, B12 methylocobalamin, methylfolate (standard MMA testing for these 2 neurological forms of B vitamins is inadequate-you may also need a CSF homocysteine test), glutathione, gastric acid (both low and high levels are associated with GERD /acid reflux), cholesterol, tissue magnesium, electrolytes and others.
--A complete iron (low iron, anemia are suspect), hormonal and neurotransmitter panel is recommended ASAP!

-Your G/I and digestive systems are dysfunctional and affecting your ability
to properly digest, absorb and assimilate vital substances, therefore, you need a thorough investigation done. But first things first. These issues could be related to low gastric acid levels.
There are a couple self tests you can do on your own as an assessment (not diagnostic).  The baking soda burp test and the Betaine HCL challenge.
Just look them up online for details.

Please try to rule out as much as possible from the above and get back to me with your results. Make sure you get  a copy of all your lab reports!
