Extremely itchy red rash on top of foot coming and going for 6+ weeks now?

On June 24th, 2016, I woke up in the middle of the night to my foot itching like crazy.  I didn't think much of it and went back to sleep, occasionally scratching it throughout the night.  When I woke up, there was a bright red rash surrounding a bite mark and I assumed a spider got me in my sleep.

I don't have health insurance so I waited it out.  Over a week, the rash faded and stopped itching.  Thinking it was infected, I used a ton of OTC antibiotic cream and itch spray.  It went away.

Now, almost 6 weeks later, it has returned.  It's super itchy and red, but overall I feel fine.  I don't go outdoors much at all, but my backyard is overgrown and I have three dogs who come and go.  Could it be Lyme Disease?

The bite mark is long healed but I can't tell if the rash is particularly bulls-eye looking and I certainly have not seen a tick.  The picture with my shoe was taken June 24th and the bare foot picture was taken last night.
