Tiny red scab turns into swollen red bump

For about a year, I've had this tiny, red scab-like thing on my wrist. It never seemed to go away. Recently, I scratched at it to pick off the scab. It then got swollen and turned into a bump. It's sore and, after picking at or, has some clear fluid in it. Now, it's red around the skin, a little yellow around the actual bump, and the red "scab" is in the center. What is this thing on my wrist?
