Still in Severe pain can anyone help!!!

Hi there,

I posted recently about my findings from a private doctor. He said I had cells in my anterior viterous and I had a large hameoredge in my temporal retina of my left eye. However the doctor could not find out why I get the constant deep eye pain and swelling around my eye and also the redness on my eye. He said the symptoms matched scleritis but he checked for the condition and also did a b-scan but couldn't find any scleritis or anything wrong with the muscles.

I'm really at the end of the road I've had this pain for 10 months now and its the worst pain i've ever experienced in my life its so severe and keeps me up at night and affects my sleep. I can't concentrate on anything I avoid almost all activities. I've already got other health problems and having this eye problem completely ruins my quality of life I don't have one. It makes me very upset and desperate.

I've tried a few pain medications in the hope they would manage the condition but none have worked so far. I've tried gabapentin, tramadol, dihydrocodiene, co-codomal and recently Pregabalin (Lyrica). I haven't had success with any of these medications most do not even touch the pain or make a difference only one which numbs it is tramadol but it has horrible side effects and my doctor said it is not safe to take for long term.

I really don't know what to do i'm completely desperate which is why I'm writing on here.

The private doctor I went to see said I should maybe have tried 60mg of steroids to see if it reduces the pain. Problem is my doctor isn't to willing to give me steroids without seeing any inflammation in my eye. I wonder if stronger drugs like immunosuppresants would help? I'm willing to try anything and i'm just scared my doctors are gonna leave it as it is.

Anybody have any suggestions? I've paid £800 to see a private doctor I can't really afford to see another one. thanks for reading.

Thank you for your reply. I recently had a fluroscien angiograph test the doctor just phoned me today telling me I need more laser surgery in both eyes and possibly cryotherapy. I'm not keen on getting both done but suppose I have to if they found a haemoredge at the back of my left eye. I feel like they are not getting down to the cause of the bleeding at the back of the eye. Neither are they dealing with the pain issue I have.

I have had an mri scan done last year of the orbits and head not sure If I had one done of the sinus it came back normal though. I had an ultrasound but it was of my eye don't know if it was the orbits.

I have had quite extensive blood work I think the only one I need done is one for HLA.

Yeah its a complicated problem hopefully they find the answer. Cells were found in my anterior viterous my doctor said that everyone has a few cells which are seen. is this true?

I still think I should be treated with a high dosage of steroids then if that doesn't work immuno suppresants. My doctor is not willing to give neither of the treatements.

A neuro opthamologist might be a good idea.

Neil,  I note you live in UK and the land of NHS.  I really don't know how to help you in that healthcare system. England has a few world class eye centers Moorsfield in London is likely the best.

This is how I would manage a similiar situation in the USA. I would have my favorite oculoplastic surgeon see you and rule out idiopathic orbital inflammatory syndrome (also known as orbital pseudotumor). You would of course need orbital, sinus and head MRIs. You would need orbital ultrasound and retinal OCTs. Your symptoms are compatible with scleritis and that can be a difficult diagnosis to make. You would also need some blood work to exclude some of the more common autoimmune diseases.

If that work up was negative I would have you see a neuro-ophthalmologist and neurologist at one of our fine medical schools. If they did not turn up anything, and I think that unlikely, then I think they would treat you empirically with oral steroids.

This sounds like a horrible problem.  After the work up described above its not at all likely that a cause could not be found.
