Pre op office visits were around $500. The anesthesiologist was covered by insurance. I think his surgery charge was around $6K per eye, but I also had vitrectomies, so I would expect a less complex case would be cheaper. The surgery charge included the replacement monofocal lenses as well as several follow up appointments.
Dr. MacKool is indeed famous and well known and experienced.

I too had problems with my lenses (Crystalens). I had both exchanged by Dr. Richard Mackool Sr. who practices at The Mackool Eye Institute in Astoria, Queens. He is world class. He does not accept insurance, so I paid upfront and got out of network reimbursement from my insurance company.
