I'm so sorry you are suffering. I'm not a doctor and know nothing about this stuff beyond what I'm learning from my own illness. Do you have a Gastroenterologist? If not, could your primary care physician get you a referral? There is an abdominal pain chart at http://www.healthorchid.com/Content/Consumer/AbdominalPain_Chart.asp?id=14 that suggests that pain on an empty stomach could be gastritis or an ulcer. Have you had an ultrasound? A HIDA Scan is a test to see how well your gallbladder is functioning. That might be something to consider. If your ejection fraction rate is less than 35%, it would indicate a problem with your gallbladder.
Doing your own research can only help. Check out the "First Principles of Gastroenterology", http://gastroresource.com/GITextbook/en/Default.htm. Check the biliary system.
Keep a log of what you eat, drink, what medications you take, your bowel habits and how you are feeling. See if you can find any patterns to the pain besides the empty stomach.
If worse comes to worse, you can always go to the emergency room. There's no shame in asking for help if you need it.
God bless and Godspeed!
