Manually evacuation of remaining diarrhea IBS w/ rectocele, any experience?

I cannot find anything so far on the internet of anyone else dealing with this. I had the rectocele for several years before developing the IBS D.  It is very disgusting and I cannot bring myself to discuss with my doctor.  I have seen a Gastro and had all the usual test and was given the IBS D diagnosis but never told them I was physically unable to complete a bowel movement without helping to clean.  It really is just too embarrassing to say to someone.  I have never told anyone. I believe because of the rectocele I cannot push out the loose bowel movement so my only alternative is to sit and wait for gravity to work which really is not an alternative.  Sometimes my movements are painful due to gas.  I've learned to avoid many foods that make this worse. It has become the norm for me.  I'm extremely careful with my hygiene knowing the dangers there.

I wanted to ask if any doctors had any experience with a case like this and if any patients here could relate.  Is there and solution for a situation like this?  I've tried medications for the diarrhea but they usually end up causing more gas, bloating and pain.  The consistency is like a paste.  Occasional water diarrhea but rarely if ever solid now for three years.
