extremely worried young man, someone please answer

Hi, my situation is...
3 weeks ago i gave unprotected oral to a stripper. In the last week especially i have been experiencing strange symptoms that i have never had before. Mainly uclers in the mouth, dry cough and a soar throat, note that i never have a soar throat on it's own, it is normally with a blocked nose ect. Also extreme fatigue, swollen lymph nodes and have lost a lot of weight. Also a few spots and a rash have appeared on my chest and neck and throat area. It feels hard to breathe and difficult to swollow, it feels like a lump is in my throat. I believe every factor was against me on this exposure. The reason why my case if different to anyone elses is because i've had a lot of trauma done to my teeth in the past year. I've had route canal surgery following an accident and permanently wear a metal band on the back of my teeth so i have had a lot of dental work done in the last year and constant cuts and bleeding are always occuring in my mouth. The other reasons im scared are because-
- while performing i cut my toungue several times as my teeth are not straight at the bottom, this may have provided an entry.
- My immune system must have very low as i was very run down at the time, had a slight cold, and had a lot of alcohol the day before and on the actuall night.
- As this occured in a very seedy strip club on the outskirts of london and it was a 30 year old latvian stripper who could not speak much english, im almost certain that she would have been involved in the sex industry and drugs ect for a long time in latvia and in london.
- It was very agressive oral.. Could definitely feel liquids of some sort present and was pretty much in full contact with her vagina for 10 minuites without stopping. I do now remember her cumming in my mouth a little too.
Questions-
Assuming she was positive and after taking these points into condsideration is it still a low risk/no risk?
Considering the UK states it's a 1, 10,000 chance can i still be fairly confident that i was never at risk?
If she was an IV drug user (i have it in my head that she was heavily into drugs) would this make transmission even easier to occur?

As this was the only sexual activity i've ever had, and im only 18 im sure people can understand my constant worry and concern. I also have to say that lack of sleep, food and not being able to concentrate on anything else has lost me my job.
It would be fantastic if anyone replied to this with complete honesty, thank you.

The DUO test is very accurate (i.e. 99%) at 4 weeks, so, IF they decide to test you for HIV, they may use that. I have no advice on which STDs you should test for- you can trust the providers at the GUM clinic to determine which tests are appropriate for your situation. But if it were me, I wouldn't even consider testing over this specific incident.

hi joggen, just wanted to say thanks again, and let you know that i have been to my GP and my symptoms are just due to a normal cold/flu. Feeling better already. However im still going to the GUM next week to put my anxiety to rest. This is mainly because of the reaction i recieved from the strip club owner. I think theres a very high chance she is positive. Anyway so next week will be 4 weeks. Do you know if they will let me test after 4 weeks for HIV? Do you know what percentage of accuracy it will be considering the conclusive result would not be until 3 months? In your experience what other STD's should i get tested for?

"Today i have coughed up some green phlem and the very bad cough, soar throat, lump feeling in my throat and general fatigue has got worse. I have also had alot of trouble sleeping and a rash all over my neck and chest area. It is exactly 3 weeks since exposure. Is there anyway these could be signs of seroconversion?"

NO STD, including HIV, produces the respiratory symptoms that you describe. Your symptoms are consistent with a community acquired respiratory infection, and my advice for you to see a GP was based on these symptoms (e.g. difficulty breathing)- not on the possibility that you have a STD or HIV. You most likely were not displaying these symptoms when you saw your GP- I can't imagine any licensed physician in the UK suggesting that a STD could be behind your respiratory symptoms.

If I were in your situation, I would put the visit to the GUM clinic on the back burner, and seek prompt medical attention for symptoms such as difficulty breathing, very bad cough, green phlegm, etc.- none of which could possibly be due to ANY STD. It's fine with me if you want to follow your doctor's advice and get tested, but I do not think pursuing your fixation with HIV and STDs while ignoring obvious respiratory symptoms is in your best interest. But only you are responsible for obtaining proper medical care for your symptoms.

And all this forum can do is give you the facts about HIV and STDs as they relate to your exposure. There was no risk for HIV from your encounter with the stripper. There was a low risk for STDs (oral gonorrhea, herpes, syphilis), but your symptoms do not resemble any STD. If you do not believe me, you are free to search MedHelp's HIV and STD forums to confirm what I have told you.

Go ahead with your plans for visiting a GUM clinic for testing, with the understanding that these tests will not address the cause of your respiratory symptoms. However, at this point, you should direct any further questions to whichever health care provider(s) you choose to see.

This is a continuation of my last post in which i stated would be my last one...
I just phoned the seedy club where i went that night and explained the situation, and asked if the girls are ever tested for STI's/STD's. The owner replied " No, our girls do not require testing as you are not aloud to touch them"
This was a very strange place and now i can be even more certain that she must have had some STI or STD.
My final question along with the one in the post above is - Today i have coughed up some green phlem and the very bad cough, soar throat, lump feeling in my throat and general fatigue has got worse. I have also had alot of trouble sleeping and a rash all over my neck and chest area. It is exactly 3 weeks since exposure. Is there anyway these could be signs of seroconversion?
Thanks for all your help.

ok the last 2 comments have helped me alot thanks joggen. I have already been to my GP, this was a week after the exposure. The doc said i should go to my local GUM and test for all STD's/STI's including HIV as he said there is a small risk. I think if i go back again they will say the same thing. So i suppose i should go to the GUM asap.
I know this is a HIV anxiety support forum, and so you are here to ease peoples minds. But can i confirm with you that you are giving your honest view on things and not just trying to ease my anxiety and make me feel better? This is my final post. Thanks

Most likely it is not related to any STD. The only STD that you could have caught that infects the throat is gonorrhea, which is usually asymptomatic. While oral gonorrhea  can rarely produce symptoms like severe throat pain, it does not produce respiratory symptoms such as cough and difficulty breathing. Also, oral gonorrhea is very rare in heterosexual men- the vast majority of cases in men are those who have penile-oral sex with other men.

However, I urge you to see a doctor regarding your symptoms ASAP, instead of trying to figure out what is causing them through Internet research. Please do so the first chance you get, and let them decide what tests you need. If I were you I would be more concerned about more serious infections like pneumonia, strep, etc. than any STD.

ok thanks, feel better after reading that.... Though today the lump in my throat has become very painful, very hard to swollow now and very bad cough. Could this be due to some other STI/STD i may have caught in the act? Should i get tested for these?

"Assuming she was positive and after taking these points into condsideration is it still a low risk/no risk?"

No risk whatsoever. It has never happened and you will not be the first person to get infected that way.

"Considering the UK states it's a 1, 10,000 chance can i still be fairly confident that i was never at risk?"

That figure is based purely on theoretical calculations and does not hold up in the real world.

"If she was an IV drug user (i have it in my head that she was heavily into drugs) would this make transmission even easier to occur?"

No, it makes no difference whatsoever.

If you remain nervous, just have a single antibody test 6 weeks after your exposure. The negative result will prove that you weren't infected.
