Condom not to the base

Hi Doc,

I had sex with a sex worker in 3rd world county. I had sex for roughly 1 min to no more than a min and 20 secs. The condom I had on was not to the base but instead 3.5/4 of the way down. After cumming I pulled out and the condom and was 3/4 off the way down and penis was soft as the condom felt lose. Please advise on how much of a risk I might have been exposed to during this sex. I have taken a PCR test 3 days after as I was recommended by specialist and am waiting for the result. Please

* Anyone who continues to post excessively, questioning a conclusively negative result or no-risk situation, will be subject to action by MedHelp. Conclusively negative results or a no-risk situation will be based upon the criteria established by MedHelp’s doctors. Action will be taken as follows:
* After excessive posting, a warning will be issued by MedHelp
* Continuing to post regarding the negative result / no risk situation will result in a 3 day suspension
* Continuing to post upon your return will result in a permanent ban.

Thanks for the response. I am not looking for anything to change the scenario around. This is just 1 detail I forgot to include in the earlier description and want to acquire how much risk I could have been exposed to.

Time to move on, you had no risk. Stop looking for something that is going to turn your no risk situation into one with risk because it will not happen.

1 Last Ques for Teak. Thanks for you help. During the encounter I had, (I am uncircumcised), the foreskin was pulled back half way till the condom protected the 1st half(from tip side) of the penis..Can you please tell me if that would pose any risk. I understand you mentinoed that all which is needed to be covered is the TIP of the penis. But just asking if the tip which usually covers the foreskin got in contact with fluids and once condom was off went back it oculd pose any risk.. Please confirm..Thanks a bunch you guys are great for the help you're providing!!!

Yes, it can. Considering the time that it was taken "but not for you". WWs need to know that there is nothing called as Negative / Positive associated with NAT tests. The detection is for the genetic material directly hence, it's either detected or undetected.

A DNA PCR is optimum at 28 days post exposure. However, YOU have nothing to worry about as it was told to you initially that you never really had an exposure and the medical practioner that suggested you to go for DNA PCR is a complete idiot because of two major reasons -

1. Primarily, no testing was warranted at the first place.

2. DNA PCR is not recommended during that time frame for early detection, it's RNA PCR.

Both the tests above are not stand alone tests and need to be backed up with an antibody test at the 12th week.

Please seek appropriate help for your anxiety, discomfort and cumpulsiveness of your throught process. HIV is not your problem and your symptoms are more related to aniety than any thing else.

Anyone very knowledgable on tests Please give me input on this. Can PCR DNA give false-negatives? I've heard that there has been very rare false-positives but never heard of any false-negatives. I know Elisa is conclusive at 3 months and its the gold standard but someone going through anxiety would definately want to take this test. Can Someone very knowledgable about this test give an accurate input. Thanks.

Ok Thanks for your input. I am forgetting about the incident till 3 months when I can take a ELISA test but my symptoms seem to get worse, fever has stopped but severe sore throat and dry cough is building up a lott. Don't understand why and it is a crazyy coincidence that this is occuring right at the 2-4 week mark which is messing with my head.

No most doctors do not go by PCR-DNA test because they are unreliable and not approved for diagnostic use.

I think the timing that might be purely coincidental is what's getting me and my doctor worried. Hopefully you are correct and I don't have nothing to worry about. For some reason most doctors I spoke to in NY go by the PCR DNA test and say if you have any infection, it'll pick it up within 72 hrs-4 weeks which is conclusive so that's why they suggested me take the test again just to be sure. People on this forum have posted that sex was protected but again as I mentioned in the earlier posts the penis was not fully covered instead till 3/4 or even 2/4 the way which is also why the scare and worry is there. Do you believe the ultimate thing to di is just get a ELISA at 3 months or should I just rely on this last test and forget about it.

The PCR test is a waste of medical resources. You had PROTECTED sex. 99+% of people who have influenza-type symptoms have influenza or some other common community-acquired virus, not HIV. You are your doctor are confusing association with causation. The timing of your symptoms is purely coincidental.

Thanks for the response. The worry that I have is that the symptoms I have are exactly 2 weeks post possible exposure and are the same exact symptoms as mentioned by Dr. Cummings in his forums of High Fever, Severe Sore Throat and Chills(all influenza type).and now developing dry coughs...I have not caught these symptoms from outside as I haven't been outside my home in 4-5 days or exposure from anyone and just got sick all of a sudden. I have asked my Dr. and recommended taking another PCR test as it can rule out any positives or negatives due to the Acute HIV symptoms that I'm having at this point.

HIV is not diagnosed by symptoms. The symptoms that HIV can produce are also produced by infections that are hundreds of times more common than HIV. A person's risk is determined by their BEHAVIOR, not by their symptoms. A small cut on the base of your penis would not serve as a route of transmission. Because you had protected sex, you do not have an HIV concern. Nobody has ever caught HIV through protected sex.

Please help. It is 2 weeks since possible exposure and I am sick with a fever, chills and sore throat.  I took the PCR-HIV test post 72 hours from national health testing as that came out negative.. A rare possiblity might be that I had a minor small cut on the base of penis from a day earlier. Please tell me if the risk exposure is more sensative as I have "possible" symptoms. Please help. Thanks.

Had the condom been that loose it wouldn't have stayed on your penis. Condoms come in all different sizes and there no reason for you to try to use a bigger condom than the size of your penis.

Ok. Thanks. I am still waiting for the result to get confirmation. If the condom is loose but the tip is covered, can the fluids still get in and cause any risk to contract any diseases. Please reply. Thanks.

NO. The only thing that needs to be covered is the head of your penis.

If the condom is not to the base, can any contact on the top part of penis(next to balls) pose a risk?

1. There are no doctors here in the forum
2. The tip of your penis was protected during the whole act and I see no reason for you to fret.
3. There is "zero" risk involved in this situation.
4. No tests were warranted.
