HIV Transmission Risk: Female - Male

I'm a 26yr White Male. I'm circumcised. I'm straight. I recently had a one night stand with a 21yr White Female, she's very promiscuous & her HIV status is unknown. We had sex on April 1st 2014. I gave her oral for roughly 8mins then we had protected vaginal sex for about 30-40mins, I took the condom off (unprotected intercourse) for roughly 5mins, so I could finish. I know CDC Risk of transmission is roughly 1-1,000 to 1-10,000 for female to male. I know it's largely dependant on viral load.

I think she might've given me HSV-1 or HSV-2 but the symptoms are very mild to non existent or I simply could be worried. I've had a few painless sores on my inner lower lip since but they've healed.  

My question is, what is my risk of hiv transmission in this situation if she even had anything?
Are the oral tests from CVS and Walgreens effective or should I have a blood test done?
Also, she's been loosing weight but that could be due to alcoholism, poor eating habits, tobacco use etc.  
I haven't slept with anyone since 2012 and I've been tested prior to 2012 for everything.

You decided to continue sex unprotected after some time. This is a mistake you committed. This act of yours puts you under the risk,,, We can not calculate the risk since HIV transmission is not a game of numbers and probabilities

Oraquick tests are reliable and accurate, provided you test after the window period

Her weight loss is not an indication of HIV, unless she get herself tested
