My son is 4year old and has undergone 2 surgery.  First bending of PAH and 2 patch to close VSD.  Now Doctor has doubt that his patch is open.  Is there any treatment for this or repeat surgery is necessary.

Many thanks for your comments.  We have now done Angiographic of our child and its finding read as -
-Fluro shows course of 5 F pigtail catheter.  FA-Aorta-LV.  Ventriculogram shows opacification of dilated, smooth walled, well contractile left ventricle, LVOT, ascending aorta, aortic arch & descending aorta.  It also shows opacification of right ventricle via large residual ventricular septum defect.
-5 F Berman catheter course IVC to RA to RV to LPA.  Injection shows opacificaiton of LPA, its peripheral branches with arborization pattern.  In levo phase there is opacification of pulmonary veins draining into LA, LV and outflow tract.

And RECOMMENDATION read as - A post operative case of DORV VSD closure.  Echo, angiographic and Haemodyanamic study shows residual VSD with QP: Qs 0.77 and calculated PVRI 6.83 woods unti.  With oxygenation PVRI goes down to 0.55 woods unit.

Pls. help.  We are in serious delimma as whether it is safe to go for surgery to close the open patch.

Many Thanks,
lata

MEDICAL PROFESSIONAL
Hello and hope you are doing well.

First your doctor may to assess the amount of leak through the patch. Further therapy will depend on the extent of opening of the patch and upon other clinical conditions, like if your son has pulmonary hypertension or not. If the opening is opening is causing clinical symptoms, then they may plan for a surgery to close the patch. It will be based on a combination of factors. Discuss various options with your consulting doctor.

Hope this helped and do keep us posted.
