Doctor won't sign copay assistance form

Hi

My medication copay is too high & I've been getting assistance through a foundation that helps pay for my medication. Recently the pharmacist informed me the claim was coming back denied. The foundation said the doctor needed to sign and confirm my diagnosis but the doctor is refusing to participate in the copay assistance. The doctor will not be paying or be a part in any way aside from signing saying I have the disease & I need the medication. I've asked the nurse over the phone & the nurse said they understood & stated the doctor doesn't participate. Is there any reason to why they won't sign & return?
