Lactulose is important to take to keep that ammonia in check. Sounds like she is in really bad shape. Is she able to get on a transplant list? Prayers to you.
I'd do my best to get her to a doctor...What about her diet? I'm sure they have told ya'll what to avoid but if not I'd get her off of any meat. I don't know if that would help right now but it might slow it down in the future.

First - if she is confused and delusional - you don't have to "fight" to get her to go to a hospital.
You can go to the courts and ask a judge for assistance to take her into protective custody. Or Title 47. Where you have medical and police assistance.
If you have a welfare office - or a police station you can start calling and asking for assistance --- ASK:
Where do I go?
What do I do?
How do I get her into the hospital - I cannot carry her - I'm not strong enough - etc.
She needs to be in a hospital if she is delusional - and is jaundiced that badly.
The police station can direct you to your next step.
They may even assist you ---- call them and ask for a WELFARE CHECK --- because your daughter isn't making "any sense on the phone" --- and she previously has been hospitalized for ESLD.
If the officer on scene determines she needs medical help - he will summon an ambulance (or he should)...
